As a Social Worker or individual who would like to nominate a young person to attend an Adventure Break, please go through the following step-by-step process below to ensure all the required forms are completed.
1. Check to see what dates we have available and the costs for the breaks. You can see what breaks are scheduled on our Activity Break Information page.
Please note: Weekend breaks are arranged by demand. We will place the young person you nominate on a waiting list until we have enough people to offer a suitable break (such as young people of the same age and /or supervision requirements). Once we have arranged a break, we will inform you of the dates, location and all other information. If we feel the young person may warrant special supervision in certain cases we will arrange to have a one-on-one Adventure Break, but this will be discussed if required.
IMPORTANT NOTICE: Before completing the application, please ensure you have read and understand the Application Contract details. You will be asked to confirm your understanding of the Application Contract on the Application Form.
